If you need replacement uniform, you need to fill in the above uniform order form. Without this, there's a chance that your uniform request might be missed off the next order so make sure you fill in the form and get it back to the office.

Please complete this form then print it, sign it and return it to Avantguard Head Office so that you can receive Statutory Sick Pay for the period of your absence.
